By creating a Nintendo Account, you can pre-register for Miitomo, and later use your Nintendo Account within the application. In the future, Nintendo Accounts will be used for the My Nintendo service that launches in March 2016.
Your Nintendo Account can be linked to other accounts, such as your Nintendo Network ID, Facebook, Twitter, or Google+ account.
You can also pick up a Platinum coin bonus in the process, by signing for Miitomo updates! The My Nintendo awards program and the Miitomo mobile game will be live in March.

Yes, just as long as you have an email address for him to do so. If he has a Nintendo Console like a Wii or 3DS that you have the ability to log into the store to purchase games an apps directly onto your console then you will likely also have a Nintendo Network ID that you can log in with or link to later.
